Notice of Privacy Practices

Last updated: October 4, 2026

Our commitment to your health information

This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. Thryvwell Health, LLC and the licensed clinicians in Thryvwell's clinical network use a secure clinical platform, MDI, for intake, clinical records, messaging, and care workflows.

This Notice applies to protected health information (“PHI”) created or received in connection with clinical care. It does not replace the clinical-network notice or consent that may be presented in the secure MDI intake or patient portal. If there is a conflict, the applicable clinician or clinical entity's notice governs that clinical relationship.

How we may use and disclose PHI

We may use or disclose PHI without separate written authorization for the following purposes, as permitted or required by applicable law:

  • Treatment. To provide, coordinate, and manage care; communicate with clinicians and pharmacies selected by the patient or authorized representative; and arrange clinically appropriate follow-up.
  • Payment. To support payment-related activities, including billing, payment processing, prior authorization, and coverage-related activities when applicable.
  • Health care operations. To operate, assess, improve, and administer the clinical services, including quality review, training, security, and compliance activities.
  • As required by law or for public health and safety. For uses and disclosures required by law and, in limited circumstances, to prevent a serious threat to health or safety or to report abuse, neglect, or other matters as permitted or required by law.

Other uses and disclosures of PHI generally require your written authorization. You may revoke an authorization in writing, except to the extent action has already been taken in reliance on it.

Your rights

Subject to applicable law, you may have the right to:

  • Request access to or a copy of your PHI.
  • Request an amendment to PHI you believe is incomplete or incorrect.
  • Request an accounting of certain disclosures of PHI.
  • Request restrictions on certain uses or disclosures of PHI.
  • Request confidential communications through a reasonable alternative method or location.
  • Receive a paper copy of this Notice.
  • Receive notice following a breach of unsecured PHI, when required by law.

To make a request, contact us at hello@thryvwellhealth.com. For existing patients, the MDI patient portal is the preferred secure channel for clinical-record requests and care messages. We may need to verify identity before responding.

Our duties

We are required by law to maintain the privacy and security of PHI, provide this Notice of our legal duties and privacy practices, notify affected individuals following a breach of unsecured PHI when required by law, and follow the terms of the Notice currently in effect.

Questions, concerns, or complaints

You may ask questions or raise a privacy concern by emailing hello@thryvwellhealth.com. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
